Appointment Types

Adjustment – Please only select this option if you are a current patient under active treatment

Botox – New and Established patients – treatment areas include: Forehead, Glabella, Number 11’s between eyebrows, Crow’s feet, and smile lines

Established Medical – Current patient appointment with Nurse Practitioner for follow up medical treatment

Established IV Therapy – Current patient who has previously received an IV therapy treatment

Filler – New and Established patients – treatment areas include: Deep lines between eyebrows, deep smile lines, and lip plumping

Massage 30 and 60min – New and Established patients – massage types include: deep tissue, relaxation, trigger point, myofascial stretching, and prenatal

New Patient Chiropractic – New patient for chiropractic examination and adjustment. We do require x-rays. If you’ve had x-rays within the last 2 years please bring the disc. 

New Patient Medical – New patient visit with Nurse Practitioner to discuss medical health concerns such as High blood pressure, Diabetes, High Cholesterol, Anxiety, Headaches, Depression, Dementia, Cold and Flu, etc. 

New Patient IV Therapy – New patient never seen for IV therapy. (If you are a current chiropractic patient that HAS NOT had IV therapy please use this option).
